The hosts had taken an early lead through Djibril Cisse’s classy strike.
But Bolton soon equalised via Matthew Taylor’s header and swiftly stole the lead through Gary Cahill’s low drive.
Woeful defensive errors then let Johan Elmander put away clinical goals either side of half time as Sunderland fell to their fifth defeat in six league games.
Many Sunderland supporters left before full-time and boos rang out around the stadium after Keane’s 100th game in charge of the Black Cats – and it was easy to understand their frustration at the club’s third straight home defeat.
Much of what went wrong was of their own undoing as Bolton moved up to seventh in the table following their fourth win in five Premier League matches.
Cruelly, false hope dawned for Sunderland after they took a 10th minute lead with a superbly-worked team goal.
Kieron Richardson snapped a pass through for Steed Malbranque who, after waiting for Cisse to time his run perfectly, laid a through-ball in for the French striker to slot past Juuski Jaaskelainen and in off the far post for his sixth goal of the season.
